Why can't I close interviews?

I keep getting interviews for jobs (think mid-front office corporate finance, some FO roles) and I keep getting positive, sometimes excellent feedback. But three times in the last three months it has happened that the other party simply drops off the face of the earth. "We want you to come in one last time," followed by nothing but silence. This is at 1 small firm, 1 BB, and 1 large buy-side institution. When I follow up, I'm just told that it's over.

No background checks have been conducted (though I'm clean and there's no reason I'd fail any BG check).

Has this happened to any of you? Anyone want to chime in with personal experience or any idea what this could be? Bad luck? Am I ugly? Do I carry a strain of foxdie that has killed my interviewers upon my leaving?
