High School Asset Management Internship

I met a financial advisor who works at an AM firm with approximately 50B CAD in AUM while he was giving a lecture in our high school about personal finance. Long story short, I got his business card and he told me to email him when I asked if I could intern as a his personal assistant in the firm.

Problem is, all this happened 5 months ago and I don't know if he even remembers me by now. I still want to email him, but I don't know how to approach composing this email. I'd appreciate any tips and things I should look out for when I'm writing this email.
