Do you care if your girlfriend smokes?
I come from a strictly non-smoking, non-drinking family. In fact, I've literally never actually seen my parents drink at the dinner table before. And they don't smoke either. I guess that background has a significant influence on me. Personally, I refuse to ask out/date any girls that smokes. It's just something I don't want to be part of my life. Now you might say I'm shallow because it's the "inside" and personality that counts. But hey, everyone's got their own bias. This is one of mine.

I dated a smoker once for a few months (study abroad -- almost every chick seemed to be a smoker). Huge turn off that made kissing unpleasant. I would never do it again.
A lot of the models, singers and actresses that you guys wank to on a daily basis are smokers... Are you guys going to stop wanking? I doubt it.
If you're dating a girl and she smokes, let her know that you don't like it because of how she smells and tastes. She might choose her addiction over you, or it might give her the boost to give it up. Either way, there is no point in being ideological about it. It's just one negative point.
As for me, I do think that it's a bad habit / addition. I really don't like the way cigarettes taste or smell, so if I can smell / taste it on a girl than it's a bit of a turn off / negative point, but I'm not going to pass over a beautiful / fun girl because she happens to have a pack of cigarettes. I can always ask her not to smoke near me / before we meet.
I think you are missing a very important point, not sure if because of lack of experience with real life physical connections or...Yeah can't think of any other reason, but here goes: Fantasizing about someone on a computer/TV screen does NOT require you to smell their stinky breath or taste their ashy lips.

Yes, please ask her to change her smoking schedule and curb her addiction and urges a few hours before she sees you and while she is with you because it bothers you but you don't want to live with it but omg you still want her because she is beautiful and fun. She will happily oblige and she might as well spray her house with industrial strength clorox before you go visit her and buy new sets of linen for those nights that you want to spend with her.

I don't like kissing smokers. It tastes bad. I like kissing girlfriends. Ergo, if i meet someone i like and we are seeing one another I'm going to ask her to curb her smoking around me, load up on mints and generally not expose me to that part of her life. I don't have to be mean about it and it doesn't need to be an ultimatum. I can't see how that's worse than just dumping a girl or refusing to see her because she is a smoker, or would you prefer that I was a passive boy and suffered in silence? Not my style.
I can reject someone's bad habit when it affects me directly, without rejecting them. It's not like smoking is an integral part of one's identity, or that I expect the women i'm involved with to be perfect. That would be quite boring.
I've never asked someone to quite smoking outright. I have had girls quit smoking around me, and sometimes they have quit altogether. Most women smokers I know, want to quit. Or at least that's what they tell me. Having said that most girls I meet don't smoke. I feel that it is seen as a bit unenlightened these days at least within my social circle.
