assuming the people you worked with are decent human beings...they will understand family tragedy and the death of a loved one...it happens to everyone eventually. So long as you are not absent for very long, i'm sure they will understand....and if you act professionally, they will take that as a good sign of how you will act in the future when under pressure situations.
I would phrase it as, "hey john, can i talk to you for a sec? i just got a call that my grandpa is about to die, and i need to go home to help plan the funeral and deal with the family issues for a couple days. I know its bad timing, but i don't have much choice in the matter. I hope my missing the last few days in the internship won't be a problem. Is there anything you'd like me to do before i leave?"
Go home dude. I know you're happy about the return offer but if they get mad about this, they might not be the people you want to be working with after all.

bro, there are some really really important events in life... If this is one of those events for you, it should be communicated professionally to your employer. If they don't understand you can easily find a new job.
I went through a similar event this year, I handed off some work, cancelled a couple meetings and arrived at the house in just the right amount of time. It meant the world to me, if I would have missed having that really important moment with my family, because I chose to read a few quarterly letters... I would have been really upset with myself.
