Pre-employment questions
I recently accepted a job offer at a F500 company, and have to do the typical pre-employment stuff - drug screening, paperwork, etc..
I have a couple of questions regarding the application for employment, so any input is appreciated.

When listing one of my previous internships, it asks for supervisor's name and phone number. I worked in PWM for Firm ABC, but the group I worked for recently moved to Firm XYZ. Do I put his name and the main phone number for the branch I worked for, or his name and the new number at the new firm, and include an explanation? Or, I can put his personal cell, assuming I get his approval. What is the proper way to go about this?

The application asks for 3 references, not relatives nor employers, that have known me for at least 5 years. I'd like to use a current co-worker, previous co-worker (from last internship last summer), and professor, but all three of these people have known me for less than 5 years. Does this really matter? It asks for their name/number/# of years known, but the only people I can put down that have known me greater than 5 years are neighbors/family friends/etc.. - I'd rather have solid references that actually know my work experience and education. Any advice here?

I went through the same process when I was filling up paperwork for my previous 2 internships (F500 companies)
1) Just put his name and the branch number you worked at since if they due do a check it would just be to confirm that you worked at the place.
2) Your references are fine. That knowing at least more then 5 years stuff is for more experienced employees who are like 30. Honestly I don't think they would want to talk with you English teacher from High school.

This is kind of a a build on the OP's question, but for two of the places I've worked in the past, my supervisor has left to a different location unknown to me. I tried to contact them previously, with no success. Should I still list their name along with my job or leave everything off completely?
