From what I've seen it is a combo of people just completely fumbling but more often than not it is actually rather random. If someone gets a live staffing as their first project and does well, they'll continue to get high-profile staffings and lock-up top bucket. Meanwhile, someone who had a failed pitch as their first project will likely not get as many reps on high-profile projects.
Yeah, so I got staffed with a team that I don't normally work with and I joined after that another analyst quit, so the project was already 30-40% done. And its a pretty high profile team in the group. I also sit in a different city than this team. So some things have been slipping, and I feel like in the light of this team, I just look like a fuck up.
If you really think it's an issue you gotta be proactive about it. Ask for a quick one on one with a team member and try to get caught up or ask how to get caught up. I imagine showing you know you came in behind and you're working to correct that is worth something, definitely more than just sitting and accepting the project isn't going well for you.

Speaking as someone who is on the analyst comp committee, no it would never ever be just one thing. If you're generally viewed as a top performer and you have a huge fuck-up that everyone finds out about, you'll still get paid well. Your overall rating is going to be an average across a bunch of people you've worked for, and if for some reason there's a lot of dispersion in your ratings (some people love you, some people hate you) then there'll be extensive internal debate to work out who's right and in what circumstances you're doing well vs. badly.
