not in IB but i interned at JPM PB and our dress code was actually more formal than the ibankers i met. PB analysts were required to wear ties and suits were commonplace, since clients would come into the office pretty regularly. people in IB did not have to wear ties; business slacks and button down dress shirt would suffice.
i dont know of any bank that requires suits as part of the dress code except for Bear Stearns and lehman. eek...

I've got a buddy who worked in the PB and a few who currently work in the IB. The PB analysts wear suits (more likely to see clients in the office) and the IB analysts typically wear business casual (button down and dress pants).

Some guys wear ties everyday, but not necessary. Of course it's nice to have suit jacket and tie handy just in case you get called into a client meeting unexpectedly...that does happen quite a bit.
While I can't speak for JPM specifically, in general, unless you are in a client facing business (ex. with Private Banking you cannot get away with business casual except on a day like today), most of the guys on trading floors, and analysts in IBD and Asset Management don't have as strict a dress code until you ascend to the higher echelons.

Business casual; although it's always good to have a matching jacket and tie handy if you get pulled into a meeting.
Two answers for you: 1) RTFM (or in this case STFF) for info about Fashion.
2) Depends on the group your in, who the group head is, what office your in and all that shit. More color on the position leads to more effective answers.
