If you don't get a business card at the end of an interview, because the interviewer says he ran out or forgot them, is this necessarily a indication that the interview went poorly?

No --- it means he ran out or forgot them. I usually give my business card out at the start of an interview, not the end.

Is it a good sign if they reply, bad sign if they don't or immaterial.
It's usually a good sign if they take the time to reply, but it means absolutely nothing if they don't reply - think of all the thank you's they receive - few are going to waste the time to respond to them.
Doesn't mean anything at all. Don't worry about stuff like that.

I definitely would not worry if you aren't getting any responses. Come to think of it, I've never received a response to a thank you note. This is not an indication that your interview went poorly; I ended up with an offer and got no replies to my thank you emails. I think it's just standard procedure to not reply; like jimbrowngoU said, they receive a ton of thank you notes.
Some people will respond to a thank you e-mail. The rest will not. Or in my case, someone from Goldman Sachs might even respond to your thank you e-mail starting with "Dear Jenny," when you're actually a guy. I think THAT was an indication of how well the interview went!
