i've been to some places where that's part of the screening process (i guess to do background checks) before handing out an offer

What about the second question?
well i assume that the answer to the second question is yes since they made all of us fill out the form after 2nd round and i assume some of those people did not get the offer.
to be honest it make sense. the spots are few and coveted. no reason to give an offer to someone only to run the background check and find it was all a lie and have to find a replacement.
It is probably worth mentioning that this is not the official background check (not asked for references, prior residences, etc.).
Presumably, after the offer is extended/accepted, I'll be asked to fill out a more substantial form.
