MD and beyond.

I've read a few things that talk about Department Heads, CEOs and other titles that are above MD, but I've never read anything that tells me how people get to these points. Besides starting a company or going to a new company how do people get promotions that take them past MD?

So true. And the bigger the company is, the more political bullshit you have to deal with. Just make sure no matter what level you are at in your career, you get in close with people higher than you. Even CEO's have to answer to the board.

I would agree with you, but then we'd both be wrong.
