not landing any interviews. Probably due to a combination of tough recruiting season and my search approach but I just want to make sure my CV isn't the reason.

i think you should take break dancing off but that's just me. everything else looks great.
Formatting could be the issue. Too much indenting. Why not just line up all your bullet points like you did in the leadership section. Take out firm specific description and titling of your deal experience. Instead, elaborate on your bullet points because some seem vague to me. What investment thesis did you develop and what was its impact? DUDE you have a % symbol....THAT IS A BIG NO. Write it out as "percent." Grammar! "Created a capitalization table computing % ownership if equity ratchets are exercise" Fix that sentence up. You have two spaces between "for a" in no name boutique. "2 products added" write out your numbers in cases like this. Ok...this could have been why you were dinged in the resume screening process.
How the hell do people get all these badass internships at non-targets? Not to say non-targets aren't capable but unless they're through connections, I couldn't get shit up until junior year OCR and I was at a target and this work experience you've got looks pretty damn legit
They really weren't that badass, I am only listing not more than 3 weeks of legit work for cumulative internship experiences for over 9 months in total.
Most of the time I did clerical work, pitches, BS. But I made sure to always do something I could put on my resume. Some of the other kids I interned with did almost nothing useful.

Duff & Phelps case competition isn't an activity, it would be more of an experience. You can leave it on the bottom, but I wouldn't categorize it as activity.

Yup, I'll have to do the same this year too despite all the time I spent preparing for SA recruiting. Sigh.
Weird thing is I'm getting a worse response rate than I did with less experience.
I agree about the indentation, it's too much. try to consolidate them, each one can be pretty lengthy. Also try to always have an action verb as your first bullet instead of describing the firm for an entire bullet. You can encorporate the description of the firm into your bullet by saying "Prepared/created blah blah blah for so and so private equity firm specializing in..." and then give a short description.

So are you guys against the M&I format for indenting?
Most of the M&I format is really good, but I just don't like the indentation. For my resume I didn't indent for specific projects and an MD in S&T stated "Wow this resume has a really good format." I assume it's just easier on the eyes as you don't have to dart them around to adjust to the indentation.
I feel your pain couchy. You have a solid resume, as do I and I also cannot get anything. However, i've given up and would rather work for a non-profit for free and make a difference than be a fucking unpaid intern at a no name bank.
