Should I "renetwork" with the same person?

About 8 months ago I got in contact with a guy who works at a decent MM bank in the UK which I really wanted to get into. We had a great chat, he asked me to send my CV over to an MD. But then, I had a big accident that made my life hell. For several months, I was back and forth between clinics, hospitals, surgery rooms, physiotherapy, etc. In addition, I lost the number I was texting/calling from, so our conversation kinda died out. I didn't think about reaching out again because I was preoccupied with sorting out my life which was falling apart. I want  to approach the same guy again, now that I am in the same city & have better experience overall. Should I do it?

If yes, how should I structure my message?

The title almost suggested that you had a falling out or did something stupid—just tell him what happened, except if there's more to the story than you're letting on
