I’ve accepted a 2025 summer internship at a BB but I’m considering deferring it to 2026 for personal reasons (not health). Has anyone successfully done this before? If so, how did you approach it, and how was it received by the firm? Would appreciate any insights!

just do the internship, by the time you get return negotiate with HR and ask whether you can return 2 years later. If not, dude, just rerecruit. Having a BB internship on your sophmore summer is only going to help you stand out.
