what do I do when I ask for time to jot down ideas and the interviewer asks if s/he can come back in a few minutes. Do I let him/her leave and come back after a while or should I start thinking on my feet and spewing ideas?

Answering the second part of your question, I don't see how they wouldnt want your best answer, and sitting there in silence is quite awkward. If they'd prefer an answer there and then, ask?
obviously let them go. it gives you a little more time and they probably just want to get a coffee or have a few deep breaths without suffocating on your cologne.

"Can I have a moment to structure my thoughts?" Instead of, "Can I have a few minutes to think it through."
Bring graph paper, an ink (like a uniball so they can see the ink) pen.
