Has anyone ever not gotten an internship because of a background check?

A friend received an offer for an internship outside of his home country (he is British) and during the background check a "police caution" came up in his background check. A caution is a streamlined way of handling minor offenses, however they include an admission of guilt. Because of this his offer was revoked.
Another friend had a DUI while on vacation and his offer was never made after the disclosure. It turned out his BAC was very high and he caused an accident including harming other people.
Unless you have serious offenses on your record or your credentials come back as disingenuous, the applicant will most likely get the job.
