How do MDs build business?

Everyone talks about going to buyside, so I may be in the minority here, but barring a life changing revelation, I want to be a career banker and make MD some day. For those who are MDs or are on their way there, how do you build business? Do you remember your first client and how you got a meeting? I'm gonna ask some of my MDs this too but they've been super busy lately and I haven't had a chance to pull them aside.
