Reference Check / BG check

Hi everyone, i dont want to overthink the issue at stake but i need some honest opinions. 
 

i  recently received an offer from a BB to work from their London office. The only problem is i changed jobs in the middle of the interview stage and therefor there is a discrepancy between the CV i applied with and what i need to write in the forms tgey asked me to complete.

my questions are : 

  1. how bad is my situation, could the offer be rescinded for that reason? 
     

  2. i transitioned from an off cycle internship to another one, does that make any difference? 
     

  3. should i mention on the application forma that i changed jobs or just stick to the one i submitted by CV with? 
     

  4. Could this be brought up during my BG/ reference checks?
