Pre-Employment Screening

So I need to fill out my info for the last 5 years of employment, and one job I had 3 years ago is worrying me because I quit during my shift (stupid and young) and I remember the month in which it happened, but I am concerned about when I was removed as an employee from this company's system and whether any discrepancy will result in not being able to get past this screening. It was just a stupid retail job with a now failing electronics retailer if anyone is wondering. This is the wording from the email:

"Be sure to have all the information outlined below available prior to logging in to ensure you will be able to complete your profile. Should there be ANY discrepancies contained in the information you provide, the investigation results will be reported as UNSATISFACTORY and you will not be able to be considered for any position at ___________ for nine months from the date the results were returned.

Current and previous addresses for last 10 years (Street, City, State and Zip Code)
If you lived outside the United States, use the "International" option and enter in at least the street name, city and state.
Current and previous employment/unemployment history for the last 5 years. All types of employment (Seasonal/Full/Part-time/Contract/Temporary) must be entered. Enter the employer's name, contact information, start and end date in MM/DD/YYYY format, salary, reason for leaving, and supervisor's name. Note, the employer must be the employer who paid you. If it was a staffing agency, enter the name of the staffing agency. If you are currently employed there, leave the end date blank."

So are they really that anal about months and things like that? I have nothing to hide or lie about. Just worried. And get info from the company is a pain and not working out.
