Rekindle an old (broken?) relationship

A year ago I interned at a mid market bank for six months. At first it went really well, but this was right before I was about to start my MFin, so once the summer rolled around, I wanted to have a good time before going back to school and got lazy. I started leaving around 5, calling in sick, etc. I wasn't too interested because I thought that after I get my MFin I can do much better. In the end, we didn't really have a conclusive discussion about the future. Now, the MFin has been all that and with graduation approaching, I figured that I need to get in touch with that bank again. However, I'm not really sure how to approach this. Any advice?
