I've got an "off the shelf" body 38R (Slim) and 28-30" waist Made to measure just isn't worth it for me, and I've been disappointed by them. Older Bonobos was really good, but everything's changed from them, and I can't vouch for them any more, but I rarely wear suits (had to today when a mid-sized Asian sovereign visited) and mine will likely last another 5-10 years.
I have had very good success with Suit Supply and Proper Cloth. Granted, both allow you to put in your measurements so at least when it comes to size there is no excuse to need more than minor tailoring
