Cover Letter Heading and Opening Salutations

If I'm applying to internships through OCR and all I have is a company name and job description, how should I address the opening salutations?

"Dear Sir or Madam" "Dear (company name)" "Dear Hiring Manager"

Also, does anyone have recommendations on the heading, given the information available to me?

I was thinking something like:

My name Address City, state, zip code Phone number Email address

Date

Dear Hiring Manager,

SB's for help with my rookie question.

I always had my name at the top, centered and large, with address/city/state/zip/phone/email on a line under it in smaller font separated by little bullet point things (fancy shit, I know) but that stuff's all up to you. After that though my typical format was:

Date

Company Name (if you have contact info) Their Contact Info (if given)

Dear [Company Name] Recruiting Team,

body

I've been told that one works pretty well, and is less stupid sounding than "to whom it may concern" or whatever else they tell you at your career services office.
