Has anyone ever heard or experienced an incident where one's WSO activity was caught by a colleague or the whole team? The closest thing I heard from a few years back, one associate was complaining about his VP on here, and apparently the VP connected the dots and confronted him IRL.

Yes. And unless you say absolutely nothing of value, it isn't that difficult to identify a user on this platform. Anonymity is not a real thing on sites like this if you are commenting in any way that adds value. The only question is how hard will someone else look? While identifying a user in this forum is a relatively easy thing to do, it does require a person to put in work. It may be relatively easy work for people with the right skillset, but most people aren't willing to do so.
I had a colleague put together the dots, my history was dug through and found one of my compensation comments. Started a little stir in the group and my MD told me to never post anything online, to which I said sure and then promptly ignored.
