value creation: playbook, tools, processes etc

Hi guys,

There have abeen a couple of questions around how ABC or XYZ fund creates value.
We can all build great spreadsheets with fantastic forecasts to justify a valuation but is there anyone here would could share some resources on how Value Creation Plans are designed and put in action?
What tools/processes /KPIs etc do PE funds put in place to measure performance and devise a growth plan.

I cant find anything convincing on the on the SEC website...

I know this is a very broad question but it would be great to see some resources on "how it is assessed and done".
