Ask MD to lunch
For a SA position is it normal to ask a MD to get coffee/lunch. Should you start w/ the lower positions first? Do MD’s get lunch/ coffee w/ interns or is their time too valuable?

Totally depends on the person/shop and the nature of your internship. I don't think you'd lose anything by asking though.
Yeah it’s normal, just ask if he’s free this week or next to go to lunch, and offer to grab lunch and you guys can eat in his office (at my old shops senior people always ate in their office)

Still, not sure if you should just go up to them and ask.. they may be correct for your firm's culture and setup.
The alternative way is to ask your supervisor/manager to set it up for you, that is common as well. Either way, they are NOT too busy for you.
