Anyone have issues returning to work after quitting for health reasons?

I am considering quitting for health reasons, I have some serious issues with my gut for the last year and can't really eat too many foods, drink anything other than water. My memory issues are impacting my work along with the insomnia. I've bounced around doctors with no diagnosis at this point. Stress from work is exacerbating my symptoms. I am going to attempt to talk to my DR next week about a medical note but I am thinking of quitting regardless if I can take medical leave or not after bonus is paid out. 

Anyone or anyone know somebody who quit for medical reasons? How was the experience looking for a job afterwards? I have enough emergency fund for 2 years or so money is not really a concern.
