Very Quick Cover Letter Question

This is in regards to on-campus recruiting.  From looking at many different cover letter templates, I am slightly confused.
First of all, do you put your own address in block format on the left or right side?  A third option is to be creative with this and do something like the below centered across the top with a line below it (much like a resume):

Name
Address, phone number, e-mail

Secondly, who do you address the cover letter to if you will not know the person.  Dear Sir/Madam?  Dear Bear Stearns Representative?
Finally, although you do not know the name of the person who will be receiving your cover letter, do you need to put a sender's address on the cover letter regardless?
