If it's a cold email to an MD that's outside of your network/circles then I'd recommend addressing them as Mr/Ms. in your first email and then see how they sign off when they reply. If they sign off with their first name (which they probably will) then use that moving forward. It's always better to be safe than sorry just in case the MD is an old school type guy
