Assuming a former alumni of your school doesn't reply, what is an example template of a follow up email asking for an informational interview?

There are a few examples on this site if you search for them. Regardless, the follow-up email should be short & sweet (couple sentences).
'Hi ABC,
I understand you must be very busy, but I just wanted to quickly follow up regarding my previous email. If you have a few minutes coming up, I would still love to set up a time to chat with you. Please let me know if this works for you, and I will be sure to make myself available.

Try something like this! Has worked for me in the past, and also reply to the initial email you sent as a nice visual cue to your recipient.
