Biggest F*** Up You've Seen At Work?

As interns across the country are gearing up for summer internships, I was wondering what are some of the biggest screw ups you've seen? Personally, the worst I've heard is that some kid from my school attempted to spend over 10k on new office chairs for the company he was interning at. While this isn't too horrible, it's really one of the few stories I've heard. So, what is the biggest f*** up you've seen in the workplace? Entry-level or experienced.

I did a summer internship at an Energy company...think Exxon/Shell/Hal/BP.

Apparently, some summer intern from the previous year was working in a research lab that was located on a top floor of this research building. This guy forgot to lock the lid of this centrifuge after he started it...long story short....somehow this centrifuge rockets up and breaks out of the ceiling and lands on some car in a parking lot....the guy was basically let go on the same day
