Hahaha this has happened way too many times
Some of the craziest things I have seen have been by older MDs (who would now be in their 70-80s) that did not grow up with tech in the old days of Blackberries. Numerous examples of reply alls with comments meant for the sender that included something derogatory about someone CCd and forwarding obvious spam that was NSFW. One of the old deal database providers would charge you something like 25 cents per cell returned in a query. One night an analyst did a search that returned the entire database and the bank got a six figure bill (not sure how it was resolved).
When I was scheduling emails, I somehow didn't notice I swapped AM and PM and ended up emailing over 50 people at 11 pm....
