Consultant resume for PE recruiting + how to find headhunters

I'm starting at an MBB in the fall and am interested in PE. Questions:

1. Does anyone have any examples of how to format a PE resume? Eg does work experience go above education yet, do university extracurriculars stay on, do I still put my GPA? Any examples (even with personal details edited out) would be great so I could refine mine!

2. How do you figure out which headhunters to contact for which firm? I know people say 'they reach out to you' but how can I be sure they do?
