Equity structre on start up pitch

Hi,

I'm currently working on my first start up pitch. Do you think I should put in the mgmt team section

1. Information about company structure? Like, CEO -> CTO -> Juniors, etc
2. Information about equity structure? Like, CEO - x%, CTO - x%, Junior 1 - x%,

Plus, do you think I should suggest investor how much equity I give up for his money? Like, if you give me 100k I'm willing to give you 15%? Or rather not include this %?

And tell me please if I'm forgetting about something in general:

1. Executive summary
2. Business description (with app mockups)
3. Goals and strategy (with risk analysis)
4. Management team
5. Market and competitive analysis
6. Financial structure and requirements
7. Appendices
