Grad Offer - Help

I just received an offer for a Grad Scheme (contract coming in the next weeks).
However, I have also received results from my exams and they were not great....
Can the job offer be conditional on a 2.1? is this likely and what would happen if I don't get a 2.1? 
Has anyone experienced this before?

Realistically, will they ask for my degree result in a couple of months after I signed the contract and check the result ?
