Healthcare Industry: Major players and how it is all intertwined

I was wondering if anyone could recommend a really good primer on the HC industry. Mostly just a broad overview of all the major players, and how money and services/products flow through this system. I have a general understanding, but it would be helpful to visualize this dynamic (so even a diagram could helpful).

There's some helpful discussions on the internet/WSO, but was hoping someone had something in particular they thought was very helpful. The reimbursement environment along with all the different players can get a little confusing.
