If you listed something like Miami Dolphins (2005-2006) or something along those lines, it would probably be considered appropriate. Be prepared for questions regarding your sport and participation in it though because it will definitely stick out. Best of luck.

Definitely mention it. D1 sports is comparable to a full time job. 30 hours/week+Travel at least in baseball.
It won't hurt, and will give you good opportunities to discuss all of what you did and how it could prepare you etc.
You can just add "(left sport due to injury)" to your resume or something similar. Readers will understand.
