Successful PE roll-ups

Can anyone working in PE/PC point out some successful PE platforms that conducted serial roll-ups?

What would you say are the main differences between successful and unsuccessful roll-ups?

Are most roll-ups/add-ons funded with a mix of cash and debt from the platform company, or the PE that owns the platform usually sponsors the deal (full or in part)?
