Has anyone ever refused you a recommendation or given you a bad one?

Title. Was thinking for business school specifically but if you have any stories from switching jobs that's relevant also. Without going into details I'm trying to cover my bases here (no fault of my own)...

I would think that it'd be obvious as to who would give you a bad review/refuse to give you one. For example, if you performed poorly at one job, you would just simply know not to ask for a recommendation from that boss rather than asking and getting a bad review. It really just comes down to gauging your relationships and people's perceptions of you based on past actions.
