When is the best time to take the GMAT?
The title says it all. For someone aiming to travel the IB -> PE -> MBA -> PE, is it best to take the GMAT late senior year, sometime during your banking stint, or during your PE stint?

The GMAT is only good for 5 years so the best time to take it is the summer after senior year, especially since you won't have as much free time to study at any other point in your career.
Would second this. Take it either summer after senior year during training (you’ll have plenty of time) or second semester senior year when you don’t have much to do. Particularly when you’re in or just out of school and in “study mode” I think you’ll score the best and not require more than a few hours a day of studying to do well.
Is the GMAT good for exactly five years after the test date? If so, when is the earliest you can take it and not have to redo it for b-school applications?
The answer is the same, end of senior year. Most business schools require 3 years of experience in order to attend, especially top tiers.
