I can't help you with finding something in Canada, but many "custom" shirtmakers actually offer made-to-measure, not bespoke. It can be very difficult to get them to tell you which they really do.
Here's the difference:
Made-to-measure is the modification of a pre-existing pattern to better fit your body. Most commonly, this involves sending in your measurements to a factory somewhere that then makes your shirt.
Bespoke is the creation of a pattern from scratch, and the shirt is usually made by the shirtmaker and his staff themselves. The advantage to bespoke is that the fit can be personalized much more precisely and with much greater flexibility. Also, there is no break in communication between the person taking your measurements and looking at your body and the person making your shirt. That is a big problem for made-to-measure: it is all too easy for something to get lost in translation between you and the factory.
Try Suits in the City. It's not a retail shop but instead an event based business. I have used them for suits, blazers and a few shirts. All of their stuff is bespoke. I think there shirts are $70, which is amazing. Just register on their website and they will let you know when their next events are.
Unless you are a VP or higher don't worry about getting custom shirts. Bespoke will produce an amazing shirt, but if you go bespoke you are going to necessarily go with an extremely high thread count, etc. Those shirts are not designed to be worn and beat up every day. If your an analyst/associate you will destroy those kind of shirts.
Go to Books Brothers or something, get the closest fit and have a tailor take it in or alter it for you. The shirts will look leaps and bounds better than anything off the rack and you wont spend a fortune.
Trust me when I say this, I have Banana Republic suits, Hugo, RL Black label, etc and the BR suits feel and fit better than some of the more expensive ones with just a little tailor work. I have been complimented on all of the suits and probably 3 people in my life have asked the brand. Save your money, people just don't give a shit.
The place where all the guys on Bay are going to now is Sprezzatura (5 King East). They will hook you up with a fantastic fitting bespoke shirt for $150 or so. I've been buying custom dress shirts for years and these guys are the best by far...awesome fit. They are younger guys running the business and a more senior dude doing the measuring so they understand the type of look/fit that younger guys want to have - a feature most other places in the city can't offer.
Spezzature is very good. Shirts are great but expensive. Suits are probably a better value - great quality & fit compared to what you get for the same price at Harry Rosen.

Never tried the following, but heard decent things: Maxwell's, and a new startup that has gotten some recent attention: www.shirtpal.com. Prices should be $55 to $100. I think Indochino also does shirts, and they also have a quick turnaround as they say around 2-wks delivery on their site.
Unless you have a body type that's difficult to fit w/ off-the-rack shirts, you really don't need to buy custom as a junior. At this stage, I think it's only justified if body-type issues or if you can get a custom shirt for roughly the same price as a brooks brother shirt (so $70/80ish or lower)

I never got it done but a friend of mine has got a customized a shirt from Stuart Warner, Toronto. Check out with their website, their range starts from $250 onwards and increases as per fabric, style and pattern.
I like this place in Richmond Hill. The fellows name is John, he goes by "The Bespokeman". He travels to Italy every 15 or 20 weeks and brings back the best Italian designer suits, Zegna, Valentino, Alexander McQueen, and many more. The place is small and very personal. I'm having a bespoke suit made for me with a vest. I chose a Prince of Wales glenn check. The lining is a very loud silk print. All done by hand. Lot's of detail to make my coleagues enivous. What I enjoy is the various fittings this "Bespokeman" fellow is giving me. He is very exact in his measuring and I see how well fitted this suit is. Very close to the body. John says this is the new look for men. I can hardly wait to see it finished. Anyway you should check it out. The website is lefirme.com or bespokeman.ca
