Cheers for the response. 

Couple of questions based on your suggestion

1. If let say for my call with HR tomorrow, if they ask "do you have any behavior problems or record from any of your pervious internship companies?", do i I just answer NO?

2. I was informed that my background check is conducted by 3rd party company. Why are they arranging this "prior service recognition details" phone call then? 

3. In case of let say for whatever the reason, they already know about this asks me "can you explain the incident with doctor's note at this company?" what would be the best answer to it?

Huge thanks man I know I look extremely paranoid and silly now.. but still appreciate your help cap.

I recommend caution here. If they ask you point blank whether there have been any issues in your prior roles there's a chance they find out / already know what happened. In this case, if you say no then you're flat out lying in onboarding / background check process Qs which is a major no go for many of the big firms, even if they were perfectly fine with what happened, provided you recognised it. Remember, Banking is a regulated activity. Not sure how things work in the States but forging a document and then on top of that lying about it speaks directly to one's honesty and integrity - both crucial elements when it comes to regulated employment. Not trying to stress you over this, just be cautious and don't underestimate potential implications. The call might (and hopefully will) be unrelated but in the off chance it's not, they will not be trying to cross check what happened and find out the truth (they already know and if its a no go for them they will just announce it), they will be giving you a chance to explain so just be honest and upfront about any issues, will work in your favour significantly. And going forward, brother, forging ANYTHING is a NO GO in Finance. Good luck!
