I didn't pass 3 months probation (100% company's fault) Put it on CV or omit?

Hi all, long story short I was hired for a role that ended up being completely different in reality vs what I was told during the interview process. I spent 3 months in vacuum trying to do something of use but it was basically a shtshow.. So 3 months later I was told that I didn't pass probation.
So my question is, do I put this on my CV and say something like we parted ways for mutual agreement (without blaming the company etc, but without blaming it on me either), or do I completely omit it and get a 3-4 months CV gap?
Both are bad, but which one is less bad in your opinion?
