I have a one:

One Interview: First ever interview for IB and it was a phone screen. They started with the "Tell me about yourself" and I rambled on for 15 mins (discussed what classes I like, my hobbies and interest and so on). Best part was there reaction was a basically a WTF over the phone.. Needless say, it was a no go. Ironically working at that same firm for the same role in same interview location now, so things work out eventually.

Candidate told us that she had brought her client list from her existing shop to the interview and was happy to hand it over, not even conditional on getting an offer.

Did not hire her as we didn't think she understood the importance of confidential information, professionalism or professional ethics. Also thought she was stupid.

Also did not take her client list. I'm an ethical man.

Interviewed for a position and made a slight stretch of the truth about a certain job from a few years ago like most people probably do. I didn't think it would be a big deal at all. Second round interview, it was brought up again by the same person but in a more specific way and I couldn't revert on what I said so I dug a further grave. Final round interview, it was brought up AGAIN unexpectedly and I ended up saying an outright lie that I regret. Pretty sure it was easily verified and got dinged because of that.
