How long after start date do medical benefits cover fertility treatments? IVF in particular?

as the title says...do fertility benefits kick in right away..or after working for 3-4 months....or what?  If you take a job with one of the banks (BofA, JPM, Citi, MS, etc...) how long do you need to wait before these benefits coverage are activated, assuming you want to do IVF as soon as possible? 

Most of the US banks offer 20-30k of coverage for IVF (it costs about 20k to do one IVF cycle between doctors visits, labs, and specialty medication)..and normal medical benefits do not cover IVF (because it is elective, and not medically necessary).
