Telling my MD about hair transplant

While I've been blessed in many ways throughout my life, those blessings didn't extend to my head. I'll be getting a hair transplant in the next few months and undoubtedly will have to tell my boss. We are a small team / company and have a flexible work policy so I plan to wfh for about ~5 weeks post surgery. Regardless, I'd still like to tell him so I don't show back up and shock him / everyone. So, my two questions are: 

(1) How do I even approach / frame the conversation? Feels incredibly awkward (2) Should I mention it to my other team members (ASO, VP) given the small team dynamic? I plan to own it when I'm back in the office, but unsure if it's overkill to tell everyone prior.

Just say you're getting a medical procedure and it's non life-threatening so they're not worried?

I would definitely tell the boss and just be honest about it. Regarding the team, I would tell them if you going from like mostly bald to a full head of hair.  Also, the team is probably wonder why are not going to be in the office for 5 weeks
