Neither of my experiences directly answers your question, but I figured I'd try and contribute anyways.

Only semi related, but I spent the first bit of my career as an actuarial analyst, and got blacklisted by Milliman (Which is probably the actuarial equivalent of a BB/MBB if there were to be such a thing). It was totally my fault. I shit the bed on a SQL skills test for an internship. Then I tried to apply for a full time role the next year. I got invited to interview, and within two minutes received a second email withdrawing my invitation to interview with no reason given. I tried applying for a few other full time roles with them and got the same thing.

More related, I threw out a hail mary application to SIG and was invited to take the initial skills test. Didn't make the cut, but I didn't think I was far off either. 6 months later they emailed me back and asked me to take the test again. So definitely didn't get blacklisted there.

My guess based on personal experience is that blacklisting has more to do with the firm's final impression of you, vs whether or not you make it to the final round.
