How often do third year analysts fail to make associate?

Offer states that if you are unsuccessful in the analyst program at the end of the third year and can't find another position (I'm assuming that means associate), you will be fired.

How often does this actually happen where a third year analyst (who isn't already leaving) gets asked to leave because they couldn't qualify as associate?

Is this a common occurrence or do you really have to drop the ball?
