PE Business Development: How to actually get to the CEO?

I wanted to get your take on how to best approach cold calling for a PE firm looking to generate new leads without having to go through a broker/banker. When do you think is the best time of day to call? How do you get around the "gatekeeper" in order to actually reach the CEO/management? What are the best follow-up practices to remain persistent but not end up with a restraining order?
