Background Check Related Question

As far as I understand, background check entails referencing past employers about the candidate. I was wondering what might raise a "red flag" for HR? I assume there is a difference between getting in physical fight with co-workers and using a cell phone during work. Any thoughts on this?

I don't think they call previous employers. I still remain close with my boss from my college days and I know they never called him. I reckon they only call if something looks suspicious.
