U5 "Damaged" After Leaving Voluntarily on PIP?

Title says it all. I was placed on a ~3 month PIP and voluntarily left a few days before the PIP ended and any action was taken. Will my U5 be "damaged" or will it be clean? Strangely enough, it's been ~2 weeks since my last day with the firm and they haven't filed it, which makes me nervous. I will add that I never formally signed it and therefore never fully acknowledged it on paper.

Has anyone run into this issue before? I've already secured a new opportunity that I will start soon, so the next opportunity is not a concern as much as the one following and thereafter. Thanks in advance.
