what are some mistakes you've made at work?

hi just curious to see what mistakes fellow bankers have made. everyone makes mistakes - how have you dealt with it at work and what changed afterwards?

I got caught beating off as a first year analyst (not trolling) and honestly it brought me and my VP closer together. It was a human experience. He walked in because the stall had one of those faulty locks that doesn't always hinge all of the way and I literally had my cock in one hand and my phone in the other (r/bikini subreddit). It's a pretty average sized wang so I think that helps. To this day I feel like if I had either A) a micro penis or B) an absolute Thor hammer things would have went differently. Especially w/ a Thor hammer because it just comes off so much more aggressive. He kind of was just like "Fuck X, jesus" and then left me to crank. Saw him later that day by the office kitchen and had to say something so I simply asked "You like that???" and he was like "No dude, I did not". Anyways, like I said, a human experience. Live and learn (to lock doors all the way).

The ones that I made early on: not proactively asking for a clear deadline, letting someone know that some thing would take longer than budgeted, clarifying things that were unclear to me assuming that as I went on it might make sense.  

General observation: I was a bit afraid of asking questions early on, thinking if I asked something dumb, that might reflect badly (insecurity primarily existed because I was the only first year analyst, who hadn't done a summer internship in that team), but you'll be surprised at HOW content-lite even some senior bankers are (folks in their 50s and 60s) and are just there on the team-page for the schmoozing part. Asking dumb questions is ok, but don't ask it twice, and definitely don't ask a dumb question or make  a silly interjection during a client meeting.
